QI & Special Projects Coordinator

Full-Time, Non-Exempt

SUMMARY:

The Heritage Health QI & Special Projects Coordinator will be a Team Leader for Practice Transformation as it relates to Quality Assurance/Quality Improvement, Risk Management and Reduction, PCMH & FTCA implementation, Community Action, and other projects as assigned. He or she will be responsible for managing defined aspects of the Heritage Health PCMH journey to successfully accomplish program goals and to support QI and community action initiatives. Duties are providing support for all practice transformation aspects of project; including development of educational curriculum, leading practice transformation efforts, providing oversight and work on the Quality Improvement and Risk Reduction activities of the clinic, providing practice transformation support services.

ESSENTIAL DUTIES:

  1. Coordinates all Defined Project Areas

  • Leads planning, development and the certification process for Heritage Health to become PCMH recognized. Once recognized works to maintain the recognition.

  • Lead Health Center staff in PCMH, FTCA, and QI requirements and on-going reporting. Support staff in their efforts toward practice transformation with PCMH practices

  • Provides expert advice to PCMH and FTCA program staff on the development, communication and implementation of key issues related to PCMH & FTCA, including:

    • Clear vision and strategy on how clinic will successfully transform to PCMH model, ensuring compliance for FTCA certification.

    • Expected timeline for clinic participants to implement agreed upon strategies.

    • Other activities and policies that support effective and measurable clinic transformation, including obtaining familiarity with clinic’s EHR (Athena) to assist with optimal use and data retrieval.
      Produces program reports summarizing key goals, accomplishments, and outcomes.

  • Stays abreast of new developments and industry trends related to PCMH, clinical quality improvement, risk management, FTCA and other assigned program(s).

  • Manages ad hoc programs and projects as needed.

  • Provides reports to senior management and board on QI efforts; assists CFO and other staff in quality reporting to HRSA, Medicare, Medicaid, UDS and health center grantors.

  • Working with senior team develops and maintains the yearly Quality Management Plan & Risk Management Plan.

MINIMUM REQUIREMENTS:

  • Bachelor’s degree in healthcare or related field; RN or master’s degree preferred

  • In-depth understanding and minimum 3-year direct experience using clinical quality improvement models and methods (e.g. PCMH, Planned Care Model, rapid cycle improvement, quality measurement and reporting) or other relevant experience options.

  • Program management skills with demonstrated experience in managing wide range of clinical and/or administrative programs with excellent organizational skills.

  • Demonstrated strong writing skills; excellent oral communication and presentation skills.

  • Relationship management and leadership skills.

  • Ability to work both independently and to collaborate with teams of individuals in diverse settings, using solution-oriented approach.

  • Expertise in using electronic health records and report generation.

  • Expertise in using computer software, including Microsoft Office with emphasis on Excel and data analysis.

Certified Medical Assistant

Full-Time, Non-Exempt

SUMMARY:

The Medical Assistant(MA) position at Heritage Health Center encompasses all aspects of the day-to-day function in the clinic. The MA works with every staff member at the health center and will be expected to provide assistance when and wherever needed. This includes patient care duties, as well as, competency in the front office coordinator position.  

MINIMUM REQUIREMENTS:

  • A High School Diploma or GED

  • Graduate of an accredited program for Medical Assistants

  • CMA certification preferred

  • At least 3 years’ experience in a medical office and one year of experience as a Medical Assistant

  • Strong communication skills, both verbal and written

  • Excellent computer skills, particularly with database management, Electronic Health Records and all software in Microsoft Office 2000, including Word and Excel

  • Demonstrated ability to work with patients

  • Ability to multi-task and manage a heavy workload. A hands-on management style and the ability to address and solve problems or issues as they arise

  • Ability to work as a team member and work effectively with diverse people

  • Demonstrated awareness of and value of cultural competence

  • Current CPR certification

  • Excellent interpersonal and communication skills

  • Organizational skills are essential

Licensed Practical Nurse (LPN)

Full-Time, Non-Exempt

SUMMARY:

The Licensed Practical Nurse (LPN) position at Heritage Health Center encompasses all aspects of the day-to-day function in the clinic. The LPN works with every staff member at the health center and will be expected to provide assistance when and wherever needed. This includes patient care duties, as well as, competency in the front office coordinator position.  

MINIMUM REQUIREMENTS:

  • Valid and Current State LPN license

  • At least 3 years’ experience as an LPN, preferred

Licensed Clinical Social Worker

Full-Time, Non-Exempt

SUMMARY:

Provides, oversees, and/or administers a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention, and/or case management services in a clinical environment which requires a high degree of independent decision-making and program administration. Leads and trains other staff engaged in related case management activities, as appropriate to the individual position.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

  • Provides counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position.

  • Prepares treatment plans, discharge plans, and follow-up care programs;

  • Provides therapeutic crisis intervention and emergency services as required.

  • Collects data about patients through interview, case history, psychological tests, and/or observational techniques; evaluates data to identify causes of problems and to determine proper therapeutic approach or referral to other specialists.

  • Consults with other therapists and related professional and paraprofessional staff

  • Provides and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems.

  • Refers clients to appropriate social service agencies for financial assistance and other required services.

  • Follows up to determine reliability of treatment used; changes method and degree of therapy when indicated.

  • Establishes and maintains case files, referrals, and other related documents for the treatment of clients; prepares related documentation for civil commitment hearings or other legal proceedings; keeps abreast of patient data to ensure appropriate treatment and care are delivered.

  • Plans and facilitates community outreach and education activities, as appropriate to the position; may serve as a liaison and representative to community organizations and schools.

  • May provide formal teaching, consultation, and in-service training to relevant professionals in immediate and proper handling of and/or referral of a variety of matters.

 

 

MINIMUM REQUIREMENTS:​​

  • Master's degree in Counseling, Social Work, or an equivalent field.

  • At least 3 years of experience that is directly related to the duties and responsibilities specified. 

  • Current State of Wyoming Licensed Clinical Social Worker (LCSW) license in good standing required or license eligible.
